Help Using Our Elation
Patient Passport

Follow the directions below to make a quick access icon on your desktop or phone to your medical records.

Open the Elation Passport Sign-in page

Click the link below and open the Elation Passport Sign-in page in a web browser (example is Google Chrome).

In the top right corner of your screen, click the three verticle dots indicating the Google Menue. Then click "Add to Home screen"

Choose the text you would like to display next to the Elation Health icon, then click 'Add'

Now click the Elation Health Icon and add it to your home screen for quick easy access!
