How to Book an Appointment

Open the Elation Passport Sign-in page

Click the link below and open the Elation Passport Sign-in page in a web browser (example is Google Chrome).

Click "Book an Appointment"

After logging in, click the book an appointment button.

Choose your Appointment Type

Scroll down to choose your appointment type, then select an appointment time. If an appointment is not available and you need to be seen same-day for a sick visit, please call or text our office directly for an appointment.
